Basic Primary Care Membership
This includes annual wellness and follow-up visits with Dr. Baxley, sick visits as needed, body composition testing when indicated, 1 annual lab panel, and 8 sessions with our health coach.
Primary Care Membership for Patients 19–25
This includes all annual and follow-up visits with Dr. Baxley, sick visits as needed.
Primary Care Membership for Patients 14–18
This includes all annual and follow-up visits with Dr. Baxley, sick visits as needed.
Premium Primary Care Membership
This includes everything in the basic primary care membership, plus:
- Use of functional medicine labs to support treatment
- GLP-1 support when indicated
- Ambulatory blood pressure monitoring as needed
Functional Medicine Support
This includes 4 visits (1 per quarter) and allows for guidance in the use of functional labs to develop treatment plans. This plan is also appropriate for patients seeking support with peri-menopausal and menopausal hormone replacement therapy, as well as with the use of GLP-1 agonist therapy as part of a comprehensive weight management program. This is not a primary care membership.
