Primary Care Membership- $4788/year or $420/month 

(Couples  $3990/year or $350/month)

This includes all annual/acute/chronic visits with Dr. Baxley, 1 annual lab panel, and 8 sessions with our functional health coach.

Primary Care Membership for Patients under 19-25- $1800/year or $150/month

This includes all annual/acute/chronic visits with Dr. Baxley.

Primary Care Membership for Patients under 14-18 $900/year or $75/month

This includes all annual/acute/chronic visits with Dr. Baxley.

Primary Care Premium Membership- $5700 or $500/month 

(For couples$4731/year or $415/month)

This includes all annual/acute/chronic visits with Dr. Baxley, 1 annual lab panel, 8 sessions with our functional health coach, sick visits as needed, ambulatory blood pressure monitoring as needed, the doctor can order functional labs/interpret/provide guidance, GLP-1 Support.

Functional Membership- $2,000/year (this is a quarterly plan)

This includes 4 visits (1 per quarter) and allows for guidance while implementing HRT or GLP-1.
