I am not in network with any commercial insurers. Visit charges are based on appointment duration. All initial visits for new patients are 60 minutes. 45 minute and 30 minute visits are only available as follow-up appointments for established patients.

Patients with Medicare B:

For patients with Medicare B, I charge rates set forth by the Centers For Medicare (CMS). Patients must pay at the time of the appointment, and will be reimbursed by CMS for approximately 80% of the visit charge. The clinic will submit claims to CMS at the time of visit, but is not responsible for estimating the timing of Medicare disbursements to patients.

Medicare B rates:

45 to 60 minute visit - $206
30 minute visit - $146

Non-Medicare B patients:

60 minute visit - $500
45 minute visit - $375
30 minute visit - $250

Children 14 and younger:

60 minute visit - $250
30 minute visit - $125

There is a $40 charge for portal messages which require complex medical decision-making.
